Online application form for CEE Kerala MDS admissions

Can I get the details to apply for MDS in government colleges in Kerala done on the basis of NEET MDS 2013?

Apply online at www.cee.kerala.gov.in
Remit the required fee at any of the branches of State Bank of Travancore using the bank chalan available with the printout of the online application. Application fee for General/Service candidates is Rs.1000/- and in the case of SC/ST candidates is Rs.500/-.Service Quota candidates seeking admission in General Quota also, should pay extra Rs.1000/-.

Completed application should be sent to the Commissioner for Entrance Examinations. Housing Board Buildings, Santhinagar, Thiruvananthapuram — 695 001, either by Registered Post/ Speed Post/Hand Delivery so as to reach him before 5 pm on 05.06.2013.
