Is there prescribed application form to apply for NMML fellowship?

I don’t think there is an application form. But the pattern is application should contain a note of about 2,000 words on the proposed project to be done with following details:-
Name
Address
Date of Birth
Academic Record from High School onwards
Details of postgraduate work and list of publications with copies of at least two recent publications.
Details of employment
Recommendations from two referees

It should be sent in an envelope clearly marked "Application for Fellowship" to the Director, Nehru Memorial Museum and Library,Teen Murti House, New Delhi-110011.
